SPECIMEN COLLECTION
Specimen type: 

Serum (preferred) or plasma.


Preferred collection container: 
Alternate collection container: 
3 mL lavender-top (K2 EDTA) tube
4 mL green/green-top (sodium heparin) tube
Specimen required: 

1 mL aliquot of serum; minimum 0.25 mL.


Special notes: 

Serum separator tubes are NOT ACCEPTABLE. Draw upright samples at least 1/2 hour after patient sits up.



SPECIMEN PROCESSING
Processing instructions: 

Allow to clot. Centrifuge 10 minutes at 3000 rpm.


Transport temperature: 

Refrigerated (preferred). Room temperature or frozen specimen also acceptable.


Specimen stability: 

Room Temperature: 4 days. Refrigerated: 1 week. Frozen: 4 weeks.


Rejection criteria: 

Serum collected in serum separators tubes is unacceptable. Moderate to gross hemolysis.

Clinical significance: 

Approximately 1-2% of individuals with primary hypertension have primary hyperaldosteronism characterized by hypokalemia (low potassium) and low direct renin. Because serum aldosterone concentrations vary due to dietary sodium intake and body position, some physicians prefer measurement of 24-hour urine concentration for aldosterone.
