SPECIMEN COLLECTION
Specimen type: 

Serum (preferred) or plasma.


Preferred collection container: 
Alternate collection container: 
3 mL lavender-top (K2 EDTA) tube
4 mL green/green-top (sodium heparin) tube
Specimen required: 

1 mL aliquot of serum (no SST) or plasma; minimum 0.25 mL.


Special notes: 

Serum separator tubes are unacceptable. Draw blood in a no-additive (red-top) tube.


Patient preparation: 

Draw "upright" samples at least 1/2 hour after patient sits up.



SPECIMEN PROCESSING
Processing instructions: 

Allow to clot. Centrifuge. Separate serum after clotting.


Transport temperature: 

Refrigerated (cold packs).


Specimen stability: 

Room temperature: 4 days. Refrigerated: 7 days. Frozen: 28 days.


Rejection criteria: 

Moderate to gross hemolysis • Serum separator tube (SST).

Clinical significance: 

Approximately 1-2% of individuals with primary hypertension have primary hyperaldosteronism characterized by hypokalemia (low potassium) and low direct renin. Because serum aldosterone concentrations vary due to dietary sodium intake and body position, some physicians prefer measurement of 24-hour urine concentration for aldosterone.
