3 mL (minimum 1 mL) bone marrow collected in culture transport medium (preferred) or sodium heparin tube (also acceptable), OR minimum of 5x5 fresh biopsy specimen submitted in culture transport medium, OR 5x5 mm formalin-fixed, paraffin-embedded tissue block
