Liver-kidney microsomal (LKM) antibodies are highly sensitive and specific for autoimmune hepatitis (AIH) type 2. The target antigen is cytochrome P450 2D6, which is localized to endoplasmic reticulum. This specific antigen is now available in an FDA-cleared EIA format that is more specific than IFA. This reactivity is not associated with AIH type 1 (lupoid AIH) or with LKM-3 positivity, which has been found in hepatitis D infection. Approximately 8% of patients with chronic hepatitis C infection are also LKM-1 reactive.
