Specimen required: 

4 mL serum (preferred); minimum 2 mL serum. Preferred specimen is serum collected in in plain red-top tube. Serum from TWO gold-top (3.5 mL) serum separator tubes also acceptable.

Include relevant clinical information, name, phone number, mailing address, and e-mail address (if applicable) of ordering physician.
