SUSPECTED TRANSFUSION REACTION EVALUATION AND INTERPRETATION

Specimen must be labeled with patient's first and last name, medical record number or some unique other identifier (e.g.,billing number, NOT date of birth). Date of collection and phlebotomist identification (full name or tech ID) must be available. Positive patient identification must be assured.
Do not centrifuge or aliquot.
Preferred specimen: Room temperature (less than 24 hours). If greater than 24 hours, specimen should be stored at 2-8°C.
Specimen must be less than 48 hours old for testing.
Tube containing gel separator. Incomplete label ID (see specimen collection note).
Immediately return bag with any remaining blood product, along with any tubing or solution running in conjunction with the blood and completed transfusion reaction flowsheet to the Blood Bank.
