SPECIMEN COLLECTION
Specimen type: 

Serum or plasma


Specimen required: 

2 mL serum or plasma; minimum 0.2 mL.


Patient preparation: 

Specimen should be drawn as a trough level just prior to the next dose or 8-12 hours after the last dose. In patients with renal impairment, specimen should be drawn as a trough level just prior to the next dose or 12-24 hours after the last dose. Patient cannot be on Digibind.



SPECIMEN PROCESSING
Processing instructions: 

Allow serum to clot. Centrifuge to separate serum or plasma within 2 hours of collection.


Transport temperature: 

Refrigerated (preferred) or frozen.


Specimen stability: 

Refrigerated: 1 day. Frozen: 2 weeks.


Rejection criteria: 

Stability limits exceeded.

Recommended trough therapeutic ranges: 
Heart failure: 0.5 to 0.8 ng/mL 
Atrial fibrillation: 0.5 to 1.1 ng/mL


Critical values/courtesy alerts:

Clinical significance: 

Digoxin is a cardiac glycoside derived from the digitalis plant. It is used to treat congestive heart failure and arterial dysrhythmias. Digoxin levels are monitored to assure adequate therapeutic levels are acheived and to avoid toxicity.
